Firearms Authority Health Assessment

Regular patients of Denmark Family Practice can book with their usual GP for a Firearms Health Assessment. This appointment does incur a private fee and is non-rebateable by Medicare.

Please provide reception section A of the assessment questionnaire when making the booking. You will need to bring part B and part C with you on the day of consultation. 

Please note your GP may need to refer you for additional tests.
